Recombinant Corticotrophin Releasing Hormone (CRH) is not available. This makes the approach to the investigation of ACTH-dependent Cushing’s syndrome different as peripheral CRH testing can non longer be performed. Desmopressin is an alternative secretagogue but is not reliable for peripheral testing due to expression of the V1b receptors in some tumours causing the ectopic ACTH syndrome.

  • the approach to the patient newly presenting with ACTH-dependent Cushing’s syndrome
  • recommendations for referral and the degree of urgency thereof
  • the advantages and pitfalls of bilateral inferior petrosal sinus sampling
  • what other options are available for establishing a diagnosis.
